The Website

The Initiative includes a customized Internet site for family caregivers, linked to from the sponsor's web site and accessed through a branded gateway page customized for your organization. While caregivers do NOT have to use the web to take advantage of the Carolina Family Caring Initiative, those who do will benefit fully. The Internet site is a greatly expanded version of the Passport. It contains:

Downloadable forms such as a Living Will; DNR request; fill-in physician directories and doctor visit log sheets; family meeting guidelines; elder-proofing checklists; care facility "score" sheets which provide you with the questions to ask facility administrators, staff and residents in order to create an "apples to apples" comparison of facility choices; and more.

A Caregiver Workbook that allows members to more easily understand their caregiving responsibilities, and find appropriate information and support. This workbook helps caregivers chart progress and store advice from the Care Advisors.
